This article explains how team contacts find, share, upload, and track Consent Forms for their team on events.vex.com. It is written for a team's Primary Coach, Secondary Coach, Financial Contact, and District Administrator.

There are two ways a participant's Consent Form can be submitted, and only one of them requires an upload:

  • Electronic form – Submitted automatically when the participant completes the form through the team's unique link. No upload is needed, and the participant's status updates on its own. Follow the steps in Sharing the Electronic Consent Form below.
  • Printed form – Completed and signed on paper, then scanned and uploaded to events.vex.com by a team contact. Only a team contact (Primary Coach, Secondary Coach, Financial Contact, or District Administrator) can upload printed forms. Follow the steps in Downloading and Uploading Printed Consent Forms below.

Electronic submission is preferred. Use printed forms only when a participant cannot complete the form electronically.

Both submission methods start from the team's Consent Forms section.

Electronic Consent Forms

Select the Copy link to Online Consent Form with prefilled Team Number button, then share the link with each participant who needs to sign the form.

To include instructions with the link, select the Copy link to Consent Form and Instructions button instead.

Forms completed through this link are submitted automatically and do not need to be uploaded.

Printed Consent Forms

Select the Download consent form and instructions button and provide a printed copy to the participant to complete and sign.

Once the completed paper form is returned, scan or photograph it as an individual PDF, JPG, or PNG file. Enter the first name and last initial of the participant the form was signed for.

Select the file from your computer, then select Upload to upload it to events.vex.com. Repeat for each participant with a paper form.

Checking Submission Status

Whichever way a form is submitted, confirm before each event that every participant's Consent Form shows as completed. A participant without a completed Consent Form on file cannot compete on the day of the event.
